What is Facial Feminization Surgery?

Facial feminization surgery, or FFS, encompasses a sizable library of cosmetic procedures performed to help a transgender, cisgender, assigned male at birth, or non-binary transgender person to achieve the typically softer facial features characteristic of a female.

The differences between the male and female facial structure comes about as a result of many years of the body's production of, and exposure to, hormones throughout all phases puberty and physical development.

How is Facial Feminization Surgery Performed?

Facial Feminization Surgery is a broad range of cosmetic surgical procedures and can include some or all of the following cosmetic procedures

From top to bottom:

Forehead and Eyes

This is one of the most important parts of facial feminization and can involve moving the forehead back, reducing the size of the nasal bridge, and shaving down large eyebrow ridges that would otherwise be too prominent.

Nose

Both the bridge and the tip of the nose can be made smaller, and the nose itself can be reshaped according to patient preferences, your cosmetic surgeon’s suggestion, or a combination of both.

Jawline (or Mandible) Adjustment

The chiseled squareness at the back of jaw can be smoother and reduced, while the bone under the bottom row of teeth can be shaved down, while the height of the chin is lowered.

Cheeks and Lips

The upper lip can be both raised up and shortened for a more naturally feminine look. Cheek augmentation is usually not required as the body tends to plush out the cheeks on its own, once post-procedural synthetic hormones start to reallocate fatty resources around the body. Where cheek implants are recommended or desired, that plush look can be achieved with either fat grafting or with cheek implants.

Hairline

As the female body facial hairline tends to hold more of a rounded shape and a shorter forehead, the hairline can be moved forward and nicely rounded.

Throat

The throat usually holds one of the most obviously visible distinctions between the male and female facial structure. Successfully operating on this area usually greatly boosts a woman's confidence and ability to present as female. The cartilage here in the Adam's apple area is reduced by a "tracheal shave," a delicate surgical procedure performed under general anesthesia with the final result being that the Adam's apple will be invisible in a vast majority of cases.

Facial Feminization Surgery Recovery

Recovery from facial feminization surgery varies on a case by case basis depending on which procedures were preformed. However, with facial feminization recovery you can generally expect:

To be able to go home after surgery
To be on pain medication for 4-6 days.
To have ace bandages on at all times for one week. These minimize swelling and aid healing.
To resume daily activities within 10 days
To return to work within 2 or 3 weeks
To avoid heavy lifting for 6 weeks’ time.
